Lizzy Care is one of a selected few companies approved to provide support to seniors and caregivers of loved ones with dementia or Alzheimer's through Medicare's GUIDE (Guiding an Improved Dementia Experience) program.

GUIDE helps people living with dementia remain safe at home while giving caregivers the support, coaching, and resources they need. Each family has a care coordinator, nurse, and dementia trained staff to check in, answer questions, and connect them to local services. At no cost to families, they provide personalized care plans, monthly check-ins, coordination of doctor visits, caregiver coaching, tools to prevent hospital visits, and referrals to community resources.

Lizzy Care enhances this program with a user-friendly engagement platform designed to make caregiving simpler and more effective, all free of charge for those who qualify for GUIDE.
